Destiny's Child utilizes the multidisciplinary treatment team approach to assist the individual in developing a comprehensive individualized service plan (ISP) to become a more independent citizen in the community.

Individualized Service Plan (ISP)

This is a written document that serves as a guide to what types of services we offer our individuals, allowing them to become independent citizens. An ISP normally includes the scheduling of medical/dental appointments and the individual's therapeutic recreational activities.

Daily Care

  • Bathing
  • Routine Skin Care
  • Grooming
  • Assistance with Toileting
  • Ambulation & Transferring of Individual
  • Medically Related Activities to Include the Taking of Vital Signs
  • Participate in the Establishment and Implementation of the Individual's Service Plan
  • Encouraging Physical Activity
  • Applying First Aid in Case of Sudden Illness or an Accident
  • Reminding Individual to Take Prescribed Medications (Supervision/Oversight of Medication)
  • Providing Watchful Supervision & Oversight
  • Preparing Meals and Washing Dishes
  • Assist Individual with Grocery Shopping
  • Assist Individual with Activities Within the Community
  • Assist Individual with ISP Goals
  • Report Problems and Progress of Individual to Physician and Others as Approved by Individual
 

Additional Requirements

The Coordinator is in constant contact with the individual, the public and the community and brings important issues before the Senior Coordinator and Administrative staff. Specialized training or one year of experience in working with a person with developmental disabilities.

Additional Requirements

Documenting and maintaining progress notes indicating changes in the individual's condition, problems that hinder service delivery, and additional needs of the individuals.


Destiny's Child Team – Companion-Sitter

Daily Care

  • Accompany the Individual to the Doctor
  • Provide Companionship & Socialization for the Individual
  • Accompany the Individual to Grocery Store
  • Meal Preparation & Serving
  • Home Safety & Management
  • Accompany the Individual in the Community for Activities
  • Routine Household Tasks
  • Ironing
  • Remind Individual to Take Medication
  • Report Any and All Changes of Individual's Health
 


Additional Requirements

  • Provide supportive care and assistance to individual.
  • Identify the needs if it is within the scope of preparation and outlined in the service plan.
  • Be able to document and follow written instructions.
  • Understand the needs and characteristics of the intellectually disabled.
